Reportable conduct by designated agency employee (any) > NSW ombudsmen
Reportable conduct by non-designated agency employee (in course of work) > NSW ombudsmen

- significant NAI: delay in presentation, inconsistent Hx, inconsistent with age (bruising in non ambulant), injuries of different ages
- pattern of non-significant harm
- behaviour risking harm
- threatened or planned harm (incl genital mutilation)
- scared to go home <5yo
Sexual (active) - act or threat
- statement of abuse
- genital trauma
- STI
- age inappropriate knowledge or behaviour
- contact with a known sex offender
- exposure to sex/sexual materials
- exposure to grooming
- prostitution
- developmental regression
- Young person with a problematic sexual behaviour (risk to others)
Psychological (active) - anything that threatens cognitive or emotional development
- exposure to disturbing behaviour (drugs, intoxication, DV, sex)
- carer mental health
- carer behaviour has persistant negative impact on social development
- consider if passive, aggressive, sleep disorder, hugs strangers, habit disorder (rocking, wetting, sucking)
Neglect (passive) - failure to provide physical and emotional nessesities
- planning for unborn
- supervision (absent/inattention)
- shelter
- food (inc NOFFT)
- love
- poor hygiene (severe nappy rash, caries)
- clothing
- delayed medical/mental health care
- school absence (6-17yo)
- developmental delay
- Relinquishing care – immediate or 3 of last 12m in voluntary care
