Complications:
- Apnoea
- hypotension
- tachycardia
- deterioration if rare pulmonary venous or left atrial obstruction
Rx
- I+V
- 1 x IVC for prostaglandin
- 1 x IVC for resuscitation of complications
- Transfer for definitive care
Dose
- if ECHO confirmed large PDA: 0.01 mcg/kg per minute
- if ECHO confirmed restrictive PDA / unknown: 0.05 mcg/kg per minute
- titrate to maximum of 0.1 mcg/kg per minute
Deterioration after starting
- ? rare pulmonary venous or left atrial obstruction
- obstructive TAPVD
- restrictive atrial septum (HLHS, cor triatriatum, severe MS/MA, or D-TGA with restrictive atrial shunt)
- > urgent echocardiography followed by interventional cardiac catheterization or surgery
