Food protein-induced enterocolitis syndrome (FPIES)
- as for FRIE below but with
- lethargy, pallor, dehydration, abdominal distension, hypotension, and hypothermia
- usually initially chronic, then once protein excluded, acute episodes with exposure usually 2hours post ingestion (v anaph usually immediate - 1-2h WITH other non GI signs)
- malabsorption, intermittent vomiting, diarrhea, failure to thrive, and, rarely, bloody stools
- Dx biopsy
- Rx avoidance of offending protein
- Px self resolves by 2yo
- blood-streaked stools
- Dx clinical, confirmed with resolution of symptoms after cessation of protein
- Px trial of reintroduction at 1y usually successful
- CMPI (cows milk protein intolerance)
